Employee ID Badges
You asked and we listened… we will be issuing ID badges to all field staff!
These badges should be with you and available at all times while on duty with NPLH patients. These badges should be visible on the employee any time you are arriving at a new patient’s house or when on patient related business outside of the patient’s home; such as a clinic appointment. If you were hired BEFORE September 1st 2019 we ask that you send a clear head & shoulders photo. If you would like; you may stop by the office and we’ll be glad to take one for you. If you were hired after Sept. 1st we will use the photo from orientation for your badge.
The photo should:
- Be recently taken
- Be against a simple background
- Have you facing the camera at an appropriate angle
- Be professional in appearance and expression
The photo should NOT:
- Be taken at odd angles
- Contain revealing clothing
- Be taken with you and other people
You may text your photo to 901-455-2062, email it to [email protected], or if you like you can upload a photo now using the form below. Once we have created your badge Chris will deliver it to one of your cases.
